- home exercise programs

Phases
Phase II cardiac rehabilitation begins within one to two weeks after discharge from the hospital. Patients receive individual assessment and instruction and participate in groups as well. Exercises include warm-ups, use of a treadmill, various stationary bicycles, resistance training with weights and other equipment, and a cool down period. Patients are closely monitored during this time. This phase lasts four to 12 weeks, depending on an individual's need.
Phase III or maintenance cardiac rehabilitation begins after discharge from Phase II or upon a referral from a physician. It is for those who want to continue a supervised exercise program.
Cardiac Rehabilitation Team
Patients in the cardiac rehabilitation program and their families are a vital part of the team. It is up to the patients to do their part to take control of their heart disease and it is families' responsibility to provide encouragement and support. Health care professionals that offer support to patients to help them achieve their maximum potential include:
- physicians
- specially trained nurses
- dietician
- pharmacist
- physical therapist
Patients will learn about their heart condition, what they can do to help the condition, and how to follow their treatment plan. They are encouraged to ask questions and report symptoms or problems.
Patients' physicians will be periodically informed of their patients' progress throughout the program.
Coverage
Most insurance programs cover all or part of Phase II. Phase III is generally not covered by insurance. If patients are unsure of their coverage, Jones Regional will assist them in finding out this information.
